Building Caring Connections in Maine

This training is funded by a grant from the Substance Abuse and Mental Health Services Administration to the Maine Suicide Prevention Program, a program of the Maine CDC.

Thank you for enrolling in this training session conducted by NAMI Maine!

The grant requires us to evaluate training effectiveness. Your participation is important. We want to ensure quality trainings that are useful to you.

1)   Please complete a brief pre-training evaluation when you register online. If you have not already taken the pre-training evaluation, please do so at the training. After the training session you will be sent an email link to complete a post-training evaluation. Once completed you will receive continuing education credits.

2)   At the training, please indicate on the sign-in sheet if an evaluator may call you in a few months for a follow-up interview. At the time of the call you may decide whether to take part.

Your participation is voluntary. You may skip any questions.  Your identifying information will be stored separately from your responses.  Responses will be analyzed by an external evaluator and reported as part of overall findings. Thank you in advance for your help with the evaluation.

For questions about evaluation activities, please contact Emily Weston at 207-287-5362
